Electronic Communications and Image Release

In order for members of our Athletic Department staff to be permitted to communicate with your daughter via telephone, cell phone, text messaging, email, social media or any electronic means OR in order for images of your daughter to be shared on our web page or social media outlets, the Archdiocesan Consent Forms MUST be completed.
 

PIAA Physical Form

All student athletes must have a physical and complete a PIAA CIPPE form in order to participate in try-outs for any of our athletic teams. The physical and paperwork must be completed no earlier than June 1,2022. Completed forms must be submitted to the Athletics Director two weeks prior to the date of the first tryout for your sport.
